By submitting this registration form, I confirm that I am either registering on my own behalf or am duly authorised to register on behalf of the other participant(s) named herein, and to provide consent for the collection, use and disclosure of their personal data for the purposes stated above.

By registering for the Community Chest Heartstrings Walk 2026 (the “Event”), I consent to the National Council of Social Service, Community Chest, and their appointed vendors collecting, using, and/ or disclosing my personal data – including my name and email address, and photographs and video recordings taken at the Event – for the following purposes:

(a) Event Administration: To process event registration and manage the Event, including pre-event and post-event communications, event coordination, post- event surveys, and related administrative activities; and

(b) Publicity and Promotion: To use photographs and/ or video recordings of me for publicity and promotional purposes related to the Event, including reproduction, publication, distribution, public display or dissemination across any media platform.

Community Chest may share the above information with its appointed vendors solely to support the administration and coordination of the Event, on a strictly need-to-know basis and only for the purposes stated above.

I understand that I may withdraw consent for the use or disclosure of my personal data at any time by contacting enquiry@HSW2026.com, and that such withdrawal may affect my participation in certain aspects of the Event where such data is required.

My personal data will be handled in accordance with Community Chest’s Privacy Statement (available at: Privacy Statement).

I confirm that I, and any accompanying participant(s) I am registering, are in suitable physical health to participate in the Event.
